The Sac and Fox Nation Social Services Department will help any federally recognized tribal member that lives within the jurisdictional boundaries and can provide the following information for any of the services in the Social Services Department.

CDIB Card
Social Security
Current Utility Bill
Current Doctors Statement (GA or Elders Program)
Letter From Employer
